MC Command Center
First on my list of Sims 4 pregnancy mods is none other than MC Command Center!
If you’ve been playing Sims 4 for any length of time, odds are super high you’ve heard of this one, and probably play with it too. It is one of my most highly recommended must-have mods for sims 4. This mod does a lot, and most notably adds great story progression to your game!
However, specific to pregnancies, I use this mod to adjust the length of my sim’s pregnancies! Once you see all the incredible sims 4 pregnancy mods on this list, you’ll understand why 3 days just simply isn’t enough. Usually, I go for 5, maybe up to 7 or 8 if I’m playing on a really long life span!
This mod also allows you to check the gender and number of babies your pregnancy sim is carrying. You can even specify these details or let them re-generate randomly. It will also tell you who the father of the babies is, so if your sim *ahem* gets around a little… you never have to doubt! lol. You can turn on teen pregnancy, set risky Woohoo percentages (so your sims have a chance of getting pregnant just from woohoo), and turn on autonomous woohoo/try for baby interactions.
I highly recommend MCCC for simmers in general, and the pregnancy control options are an awesome bonus! I honestly don’t know if I could play my game with this one—I love checking what baby(s) my sims are having, or checking if they’re pregnant immediately after woohoo! That’s why this is one of my essential sims 4 pregnancy mods, hands down!

Lumpinou’s Relationship and Pregnancy Overhaul Mod (aka Sims 4 Woohoo Wellness Mod)
This mod is a doozy! It is hands down the most essential sims 4 pregnancy mod on this list! If you’re looking for more in-depth detail, realism, and gameplay, look no further than Lumpinou’s pregnancy and relationship overhaul mod, formerly known as the woohoo wellness mod!
I’ll be honest, I could dedicate an entire post to this mod alone! lol. It’s big. I highly, highly recommend Lumpinou’s mod description here for the full info. You can pick and choose which modules you would like to have in your game. I personally use the entire mod, but I’ll walk you through some of my favorite modules for realistic sims 4 pregnancy!
Sims 4 Pregnancy Preferences Mod
This module gives every sim a preference for if they want children or not. You can force change it, if you don’t like what they’ve been assigned, but I love the drama this creates! It’s also so much fun to see a sim’s preference change through gameplay (if you have that setting turned on). I had a sim go from not wanting kids to wanting them after they got married, which I thought was super cute!
But this also adds reactions to the rest of the family! Grandparents and siblings now have opinions. And they won’t always be positive anymore! This module adds so much realism, so i highly recommend adding it to your collection of sims 4 pregnancy mods if that’s what you want.
Sims 4 Fertility Mod
This module adds realistic fertility to the game. Some sims naturally have higher or lower fertility, and can take actions based on those things. This module also introduces birth control (could be redundant, if you’re using WonderfulWhims!) and the ability to even get sterilized if your sim doesn’t wanna risk it!
Sims 4 Pregnancy Paternity Mod
This module adds realistic paternity to the sims 4! And wow, talk about the DRAMA in this one! lol. If your sim is woohoo-ing around, they might not know who the father is, and can request tests from each suspect! Or, your male sims can admit to their partners if they impregnated someone else! 😬 Like all of lumpinou’s incredible mods, there’s custom moodlets for every single thing and tons of new interactions!
Sims 4 Teen Pregnancy Mod
The teen pregnancy module is the most in-depth teen pregnancy mod out there! Definitely an essential sims 4 pregnancy mod if you like playing with drama in your game, that’s for sure! If your teen sim gets pregnant, when they announce to their parents, their parents can either choose to support them or reject them! They can even kick them out of the house! This module comes with a lot of super realistic interactions between teens and their parents, and tons of moodlets. A must-download!
Sims 4 Pregnancy Side Effects Mod
This module makes it to where your pregnant sim can have mood swings, food cravings, and back pain. And their partners can even respond to some of these feelings! This is such a sweet add-on for realism and additional gameplay, and some bonding moments between your soon-to-be parents!
Other Modules
Like I mentioned earlier, I personally play with the entire mod! There are also modules for pregnancy termination, pregnancy loss, cheating, an in-depth separation/co-parenting module, etc. Some of those topics can be triggering for some people, or just too serious/mature for your gameplay—don’t feel like you have to use the whole mod if you don’t want to! All-in-all, whether you use one or all the modules, this is an AMAZING addition to your sims 4 pregnancy mods collection!
Realistic Healthcare and Pregnancy Risks (Adeepindigo’s Healthcare Redux)
Adeepindigo’s Healthcare Redux mod is one of my favorite sims 4 pregnancy mods for medical realism! This mod was featured on my round-up of best realistic sims 4 mods, but today, I’m specifically talking about how it impacts pregnancies.
With this mod, your sim can now go to prenatal doctor’s visits! They can take prenatal vitamins to reduce risks of complications, and can even have an emergency c-section! Your sims can even pass away from complications. 😬 (Although odds are very low!)
I made the grave mistake of playing with this mod in a Decades Challenge save file, and thought it wouldn’t be era-appropriate to send my sim to the doctor when she had a postpartum hemorrhage, and she died. 🥲
However, one of my favorite realism features in this, which really secures its spot on my list of top sims 4 pregnancy mods, is that now all sims have a chance for getting the “baby blues” and postpartum depression. I know a lot of us play the sims for escapism, but I love the representation of realistic mental health in this game too. 🤍
Sims 4 Ultrasound Mod by LittleMsSam
This is one of the smaller sims 4 pregnancy mods on my list, but it’s just too adorable not to include. This sims 4 mod by LittleMsSam lets your sis go for an ultrasound scan! When they come back from the appointment, they will have a photo in their inventory, which can be displayed.
The ultrasound will tell you how many babies you’re having, and what the genders are. I love displaying ultrasounds on the walls in my sims’ nurseries! 🙂

Small Sims 4 Pregnancy Mods by LittleMsSam
The Small Pregnancy Tweaks Mod by LittleMsSam is another small yet essential item on my list of sims 4 pregnancy mods. This mod lets you make your sim’s pregnant belly just a little smaller (or there are packages that allow for a bigger belly, if you’d prefer that), allows you to get rid of the pregnancy walk style, and allows more activities for pregnant sims.
You best believe I have my pregnant sims doing yoga alllll the time now! lol.
Wonderful Whims Sims 4 Mod (For Realistic Periods, Fertility, and Birth Control)
WonderfulWhims is one of my essential sims 4 mods, but it’s also one of my essential sims 4 pregnancy mods! Most people are familiar with the fact that WonderfulWhims adds an attraction system, but it also has a lot of realism added to pregnancies in sims 4.
It adds realistic periods for your sims. You can set fertility chances to adjust based on where your sims are at in their cycle (you can also turn this off, if it’s frustrating). Your sim can take fertility tests to check their likelihood of getting pregnant on any given day.
If you’re trying to avoid pregnancy, your sims can also take birth control! Both the pill and condoms are included in this mod. You can also turn on labor puddles, so it looks like a sim’s water broke when they’re ready to give birth!
This a great mod for more realistic pregnancies in sims 4, and I highly recommend this one to any player!
Realistic Life and Pregnancy Mod
The Realistic Life and Pregnancy mod adds tons of new social interactions, reactions and moodlets, earning it a spot on my must-have sims 4 pregnancy mods list! This mod is great for general family play, but for realistic pregnancies, my favorite thing about this mod is it gives your sim the ability to have a preferred gender and number of their babies! 🙂 Such a cute, small, realistic detail—I love it!
And there are so many great interactions! In addition to that, your sims can discuss adoption, talk about a new sibling with their existing kids, worry about providing for their baby, have tons of new pregnancy side effects interactions (like talking about the baby kicking or morning sickness), and children can even ask for a sibling! Too freaking cute.
One of my favorite sims 4 pregnancy mods—it truly enhances family gameplay! 🙂
PandaSama Realistic Childbirth Mod
PandaSama’s Realistic Childbirth Mod is one of the absolute best mods out there right now—of course it had to earn a spot on my round-up of sims 4 pregnancy mods! It is the best of the best of sims 4 pregnancy mods, in fact!

This mod is an incredible enhancement to the in-game pregnancy system, starting with the very first pregnancy test! There is a brand new pregnancy test animation, which will show your sims reaction to the news and even a little pregnancy test! There are also brand new animations for sharing the news with your partner! There are multiple reactions, depending on if your sims are happy for the news or not, but oh my goodness, my heart bursts every time at the happy-sway one. ❤️
PandaSama has also added pregnancy-specific interactions. Your sims can now nap on the couch with their partner, and it is the cutest thing I have ever seen in this game, lol. Your pregnant sim will rest a hand on their belly! Your sims can also cuddle in bed, and stare at that baby bump with awe! ❤️
PandaSama, the absolute legend of sims 4 pregnancy mods, is even working on the next update right now and plans to add a ton of more pregnancy interactions! Definitely keep an eye on this one—it’s a must-have!
Baby Shower Mod (Base Game Compatible)
This was one of my essential sims 4 pregnancy mods before Growing Together came out, but I’m still including it for those of you who don’t own the expansion pack! This mod adds a baby shower event you can throw for pregnant sims!
I absolutely love custom events in my game—they add so much realism and storytelling potential. I was so excited to see baby showers launching officially with Growing Together, but this mod is still a fantastic option!
